Membership Terms and Conditions

These Membership Terms and Conditions (“Terms”) apply to all membership plans offered by Spine Studio LLC d/b/a Spine Studio Chiropractic (“Spine Studio,” “we,” “our,” or “us”). By enrolling in a membership, submitting payment, or using a membership plan, you agree to these Terms.

1. Covered Members

A membership may cover the primary member, spouse, and any dependents under age 25 living in the same household who are added to the plan and/or billed under the same account. Any add-on members must be approved by Spine Studio and must remain on the same plan type and billing account as the primary member.

2. Membership Program

Spine Studio offers membership plans designed to provide access to chiropractic services according to the terms and visit parameters of the selected plan.

Membership rates are subject to change. Spine Studio reserves the right to modify rates based on operating costs, promotions, client needs, special circumstances, market conditions, or other business considerations. Our goal is to allow active members to maintain a discounted rate relative to pay-per-visit rates and, in some cases, future standard membership pricing, provided the membership remains active without interruption. If a membership is stopped or cancelled for any reason, any future re-enrollment will be at the then-current rate in effect.

3. Membership Is Not Insurance

This membership is not an insurance plan and is not a substitute for health insurance. Membership fees may not be billed to insurance. This program is intended to provide access to chiropractic services without the restrictions of third-party payers.

Spine Studio does not provide insurance billing codes for membership fees, but receipts may be provided upon request. In some cases, HSA, FSA, or similar benefit plans may reimburse eligible expenses. It is your responsibility to determine eligibility for reimbursement.

4. Recommended Care

Membership enrollment does not replace clinical judgment. A recommended care plan may be made regardless of the membership plan selected, in accordance with applicable professional standards and Texas law. The frequency of visits recommended by the chiropractor may differ from the number of visits included in a membership plan.

5. Discounted Fees

Membership fees are discounted fees and may be lower than Spine Studio’s regular fees for individual services. These discounted fees are available only while the membership remains active and in good standing.

6. Services Covered and Not Covered

Unless otherwise specifically stated, membership covers chiropractic visits only within the descriptive parameters of the selected plan. Other services, products, therapies, retail items, or fees are not included unless expressly identified as part of the membership and may require separate payment at the time of service.

If offered, discounts on other services or products are separate from the membership and may be changed or discontinued at any time.

Any visits occurring outside regular office hours, on holidays, or under special circumstances may be treated as outside the normal scope of membership and may incur an additional fee.

7. Change in Health or Injury Status

You agree to notify Spine Studio within 72 hours if care becomes related to a work injury, automobile accident, personal injury claim, or other third-party liability matter. Spine Studio may choose not to participate in reports, testimony, legal opinions, or litigation support unless required by law or separately agreed in writing.

All healthcare decisions remain your responsibility.

8. No Long-Term Contract; Recommended Initial Commitment

There are no long-term contracts, early termination fees, or financial penalties for cancelling a membership. Spine Studio requests an initial commitment of 3 months, but you may stop the membership at any time by notifying the office by phone, email, or in person.

Spine Studio requests at least 5 days’ notice before the next billing date to stop future recurring charges. If notice is received too close to the billing date, the cancellation may take effect on the following billing cycle.

Spine Studio reserves the right to decline future membership enrollment after cancellation. If you later re-enroll, the membership will be at the then-current rate and terms.

9. Return to Membership

If you cancel and later wish to rejoin, you may enroll under the rates, policies, and terms in effect at the time of re-enrollment. Previous rates, promotions, or discounts are not guaranteed after cancellation or lapse in membership.

10. Office Closures and Schedule Changes

The doctor and staff may be unavailable from time to time due to vacations, holidays, seminars, illness, emergencies, or other scheduled closures. In addition to standard holidays, the office may occasionally be closed on other dates.

You are responsible for maintaining a current email address and contact information with Spine Studio so that notices of closures or schedule changes can be provided. If in doubt, please call the office before coming in.

11. No Guarantee of Results

These Terms relate only to membership access, pricing, and payment terms. Spine Studio does not guarantee any particular outcome, improvement, or result from services provided. No promise has been made that any condition, illness, injury, or disease can be prevented, corrected, or cured through participation in this membership or through chiropractic services.

Charges incurred under a membership are for access to services and benefits under the membership and are due regardless of outcome.

12. Non-Transferability

Membership is non-transferable except to approved household members covered under the same account and plan. Membership may not be sold, assigned, or transferred to another person or another chiropractic office or facility.

13. Electronic Payment Authorization

For your protection, payment information is stored and processed through authorized third-party payment systems and is not retained for direct access in the office.

By enrolling in a membership, you authorize Spine Studio LLC d/b/a Spine Studio Chiropractic to charge the payment method provided, including credit card, debit card, ACH, or other approved payment method, for:

  • the selected membership fee charged today, if applicable;
  • future recurring monthly membership fees on the same day of each billing cycle or nearest practical processing date; and
  • any authorized membership-related charges, declined payment reprocessing, or applicable returned payment fees.

Recurring billing will continue until cancelled in accordance with these Terms.

14. Payment Terms

By enrolling, you understand and agree that:

a. the selected membership fee will be charged on a recurring monthly basis unless and until cancelled;

b. membership pricing, included visit parameters, benefits, or related terms may be modified by Spine Studio from time to time;

c. the membership provides access to chiropractic care, checkups, care recommendations, and related services from the chiropractor as described in the selected plan;

d. the chiropractor may recommend care frequency based on clinical findings, symptoms, history, goals, and other relevant factors, regardless of membership level; and

e. additional services, therapies, products, or visits beyond the scope of the selected membership may require additional payment.

15. Cancellations and Refunds

You may request cancellation at any time by phone, email, or in person. Spine Studio requests at least 5 days’ notice before the next billing date.

If the membership plan provides a specific number of visits per billing cycle and you cancel during the current billing cycle, you may request a prorated refund for unused services in the current billing cycle only, if applicable and if required by governing rules or policies. No refunds will be provided for prior months.

If membership fees are charged for access to membership benefits over a period of time rather than tied strictly to a set number of visits used, no refund will be issued for prior months regardless of usage.

Any refund rights are limited to the current billing period only and are subject to applicable law and Texas Chiropractic Board rules.

16. Declined Payments / NSF

If an ACH or other payment is rejected due to insufficient funds, declined card, or similar payment failure, Spine Studio may reattempt the charge. Spine Studio may charge a returned payment or NSF fee not to exceed $20 where permitted by law.

By submitting payment information, you represent that you are an authorized user of the payment method provided and agree not to dispute charges that are consistent with these Terms.

17. Responsibility for Accurate Information

You are responsible for providing and maintaining accurate contact, household, and payment information. Spine Studio is not responsible for errors, missed notices, or billing issues resulting from outdated or inaccurate information.

18. Modification of Terms

Spine Studio may update or modify these Terms from time to time. Continued participation in the membership after any such update constitutes acceptance of the revised Terms to the extent permitted by law.

19. Entire Agreement

These Terms constitute the full understanding between you and Spine Studio regarding the membership program and supersede prior discussions or representations relating to membership terms, payment terms, or included benefits, unless otherwise agreed in writing.

20. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Texas.
